Enrollment Process
ICICI Lombard Insurance Co. Ltd shall provide enrollment forms.
Deputy Registrar shall distribute forms to every cooperative society.
Every cooperative society shall mandatorily fill the enrollment data in the format as provided along with instructions.
After completion of the enrolment, each cooperative society shall forward all the enrolment forms along with enrollment data of its society to its respective Deputy Registrar of its district.
